Discharge Process

Helping you to a speedy recovery.

The process begins on the day of admission to allow early recognition of potential problems that may delay discharge and to ensure that appropriate services are available if required.

You should plan to leave your room by 10am. You are welcome to utilise the lounge or gardens while you are waiting to leave the hospital. Prior to discharge please collect:

  • Any valuables held in safekeeping
  • Any x-ray copies, medications, etc
  • Any aids, frames, etc, prescribed by the therapist
  • Details of doctor’s appointments, or community assistance that may be required
  • An account for incidentals not covered by your fund (such as telephone calls or equipment to assist mobility) will be calculated and an invoice given. Please settle your account prior to leaving the hospital.

A Discharge Summary with all the relevant information will be forwarded to your Specialist and to your General Practitioner on discharge.

Home Visits

A home visit will be conducted, if appropriate, to assess safety within the home. The Occupational Therapist consults with the patient and or family and documents any recommendations. If the recommendations are accepted, the therapist will coordinate all necessary aids and modifications to the home, in consultation with the family.
