Ruth CThe process begins on the day of admission to allow early recognition of potential problems that may delay discharge and to ensure that appropriate services are available if required.
You should plan to leave your room by 10am. You are welcome to utilise the lounge or gardens while you are waiting to leave the hospital. Prior to discharge please collect:
A Discharge Summary with all the relevant information will be forwarded to your Specialist and to your General Practitioner on discharge.
A home visit will be conducted, if appropriate, to assess safety within the home. The Occupational Therapist consults with the patient and or family and documents any recommendations. If the recommendations are accepted, the therapist will coordinate all necessary aids and modifications to the home, in consultation with the family.
