At Alwyn Rehabilitation Hospital we value and welcome your feedback.   Your feedback, both positive and negative, can help us learn about how we are going at delivering patient-centred care, to help us understand what we’re doing well and what we could perhaps do better.  Please help us provide better care by completing the feedback form below.  If you would like a response, please ensure you indicate this in your email.

If you would like to lodge a formal complaint, please address this to the CEO/Director of Clinical Services. This will be reviewed by Management.

If you are not satisfied with our response, you can refer the matter to the Health Care Complaints Commission on 1800 043 159, or on their website at www.hccc.nsw.gov.au.
