Pre-admission

Excellent Outcomes for our Patients.

• The patient is referred for pre-admission (by telephone, fax or email) by NUM / Discharge Planner / MO.
• The Director of Clinical Services (DCS) assesses the patient (may include a site visit).
• If the patient meets our admission criteria, the DCS arranges the Health Fund Check and liaises with the referring facility to confirm bed availability.
• The REFERRER informs the patient of any co-payments, whether the confirmed bed is in a private room or shared ward, confirms bed with DCS on the day prior to transfer, and arranges transport to Alwyn.

If the patient does not meet admission criteria, the referral is declined.
